Appointments and Referrals
Please read the referral and appointment information below before sending us your appointment request or enquiry:

For referring offices
New patients are seen by referral only. The practice is open to both adults and children. Referrals must be faxed to 416 864-3033. If the appointment is medically urgent, doctors must clearly indicate this on the referral note. Dr. Vadas reviews all requests for medically urgent consideration.

Our office contacts patients directly with appointment information. Doctors' offices needing confirmation that an appointment has been scheduled, please indicate this on your referral note. We will fax back confirmation after the appointment is booked.

For patients
Our office experiences a very high volume of telephone traffic. For this reason, we encourage e-mail appointment booking and enquiries. Please do not e-mail confidential medical information.

Due to the very high volume of e-mail and telephone requests, we ask that patients EITHER e-mail about appointments, OR telephone - please, not both.

Dr. Vadas does not provide test results, medical advice or prescription repeats by telephone, fax or e-mail. Patients may schedule an appointment with our office for these services, or contact their family doctor.

ABOUT YOUR APPOINTMENT

The Cardinal Carter Wing is on Victoria Street, one block east of Yonge at Queen. Please enter through the Victoria Street doors.

Duration
Initial visits that include allergy testing are approximately 1-1/2 to 2 hours. If subsequent visits are required for additional testing or to obtain test results, you will be given an approximate time when the visits are scheduled.

Please bring ...
(1) a list of medications that you are taking
(2) your health card
(3) the names and complete mailing addresses of physicians you wish reports sent to

NOTE: If your visit will include allergy testing, please remember to take NO ANTIHISTAMINES for 3 days prior to the appointment.

Cancellation / Rescheduling
We understand the need to reschedule appointments for personal or employment-related reasons. We appreciate as much advance notice as possible, to enable us to offer your appointment time to another patient. Patients needing to reschedule will be contacted with the next available appointment. (Note that we are now scheduling 5 to 6 months ahead.)

Cancellation list
Patients must be available on short notice (3 to 4 hours), and must not have taken antihistamines for 3 days prior, in order to be considered for a cancellation time slot. To add your name to the cancellation list, click on CONTACT CHRIS, and include a contact telephone number. You will be contacted by telephone with an available cancellation. Appointments will not be held open - if we are unable to reach you in person, we will contact the next person on the list.
